§ 17-48. Physical examination and report prerequisite to issuance; forms of reports.
(a)
Every applicant for a driver's permit shall submit himself to some physician practicing and maintaining an office in Volusia County to determine whether or not such applicant is suffering from a contagious or communicable disease and whether or not such applicant has any defect or impairment of vision or hearing or other physical impairment which would render him incapable of driving a motor vehicle safely.
(b)
The report of such physician duly sealed shall be submitted with the application for a driver's permit.
(c)
The chief of police and the city clerk may jointly prescribe forms on which such reports shall be submitted.
