§ 17-28. To whom application; contents.

Applications for permits shall be made in writing to the city clerk and such application shall set forth:

(a)

The name of the applicant.

(b)

The proposed place of business of the applicant.

(c)

The kind, make, model, motor number, state license number and passenger capacity of each vehicle to be used in the business, together with a statement of the ownership of, and liens against, each such vehicle.

(d)

If the applicant is an individual, the applicant's residence address.

(e)

If the applicants are co-partners, the names and residence addresses of each co-partner, and the name of the co-partnership.

(f)

If the applicant is a corporation, the application shall set forth the name of the corporation and the residence address of its officers and board of directors.

(g)

The name which will be used in designating the taxicabs and the taxicab business of the applicant.

(h)

A sworn financial statement of the applicant.

(i)

The date on which the permit applied for is requested to become effective.

(j)

The maximum number of taxicabs which will be operated.

(k)

Any other relevant information which the city clerk may require.
